Emergency Information Card

Parents, as you know it is extremely important that you can be contacted in the event of an emergency. This year, we are placing the blank emergency information card on our website in an effort to assist you. Please, print the card, fill it in and bring it with you during our Student Schedule Pickup Day on Thursday, August 16, 2007 in the cafeteria. This will streamline the process. We require a completed emergency card before you can pick up your child's schedule.

Parents, please include a "Parent's Email Address" on the Emergency Info Card. Dr. Gracia sends out a lot of information directly to parents throughout the school year who have shared their email addresses with us.

If you are unable to attend, please send the completed form with you for your child's first day at school. Thank you.
